Are Some Tourniquets Better Than Others?
Yes—by a massive margin. Dedicated, commercially manufactured tourniquets recommended by the CoTCCC (Committee on Tactical Combat Casualty Care) are significantly more effective than improvised alternatives.
Recommended Commercial Tourniquets
C-A-T (Combat Application Tourniquet): The standard issue for the US Military and most emergency medical services. Uses a internal free-moving band with a durable windlass.

How to Apply a Tourniquet (Step-by-Step)
If someone is suffering from life-threatening bleeding on an arm or leg, speed is everything.
Expose the wound & assess: Immediate.
Wipe away excess blood to identify the source. If bleeding is massive and spurting, apply a tourniquet immediately.
Position the band: High & Tight or 2-3 inches above the wound.
If the exact origin of bleeding isn't clear, placing the tourniquet high and tight on the limb (top of the arm or thigh) as quickly as possible is recommended. If the wound is clearly visible, place it 2 to 3 inches above the wound directly on skin or over thin clothing. Never place it directly over a joint (elbow or knee).
Pull the strap tight: Eliminate all slack.
Pull the main strap as tightly as humanly possible before locking the mechanical windlass. Eliminating initial slack is the most crucial step for stopping arterial flow.

How to Make an Improvised Tourniquet

To create an effective improvised tourniquet, you need two items:

Myth 1: "If you put a tourniquet on, the patient will lose that limb."
Reality: Surgical teams routinely use tourniquets for hours during orthopedic procedures. Clinical data from combat zones and civilian trauma units show that a tourniquet can remain in place for up to 2 hours with minimal risk of permanent nerve or muscle damage. Limb loss is almost always caused by the original traumatic injury, not the tourniquet itself.
Myth 2: "You should loosen the tourniquet every 15–20 minutes to let blood flow."
Reality: Never loosen a tourniquet once applied. Periodically loosening it allows rapid arterial re-bleeding, which can quickly cause hypovolemic shock or death. Only qualified medical professionals in a clinical setting should evaluate and adjust or remove a tourniquet.
